Everyone seems to recommend using 1/2" pex for plumbing systems. Does 1/2" refer to inside diameter or outside diameter? same question for all the fittings etc.

The od of PEX is the same as the equivalent size of copper tubing.
One of the many benefits of PEX is fewer (leak free) joints. Poor workmanship is simply that. I’ve used a lot of PEX and have never seen a leak anywhere but maybe I use the proper procedures and tools when working with it.
