I have 23,500 miles on my van. I'm experiencing shakes when I brake on high speed. I also hear some brake noises occasionally. When I rotate my tires I notice the rotors are all grooved and badly worn. What's going on my brakes? Is this the warranty issue? Thanks.

It sounds like one or more calipers, the section holding the pad set and hydraulic piston, are not 'free floating' on their 'self-centering' slider mounting guides so even a little rotor warp translates to shudder, anyway its not getting full friction at that wheel. Used to happen almost every year on my F-150 4x4, a little prybar action to break it free and it was fine for another year - those sliders must be well greased to keep sand/grit/corrosion away, I knew mine were so it was probably slightly mismatched rebuilds from when I'd replaced the brakes a few years back...

On the disc rotors being grooved/worn... it depends.
